If your child needs prescription medication whilst in school we will require a completed Medication care plan, before we are able to administer medicines. Medication care plans are available on request from reception staff, or can be requested via your child's home-school link book or downloaded below. Please note that we will need the completed, signed form to be returned to us before medicines will be administered.
